BACKGROUND:
San Mateo County is responsible for providing necessary medical care to its medically indigent population. Dr. David Shields has provided gastroenterology services for San Mateo Medical Center since 2019 and at SMMC's request, has increased the quantity of his services over that time period.

DISCUSSION:
Under this agreement, Dr. Shields has agreed to further increase his services in the area of call coverage, while maintaining a similar level of clinical services at a minimum of six half-day clinic blocks, during which he will see patients, consult, and perform procedures, including endoscopic retrograde cholangiopancreatography. Under this agreement, Dr. Shields will provide a minimum of 180 clinic blocks in endoscopy, including ERCP, to serve patient needs. His call coverage amounts are increasing from 88 weeknights to 96 per year, from 12 weekend calls to 16, and from 2 periods of holiday weekend call per year to 4.

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

The agreement and resolution have been reviewed and approved by County Counsel as to form. This agreement is coming to your Board late due to prolonged negotiations.

The resolution contains the County's standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
