Adopt a resolution authorizing an agreement with David Shields, MD to provide gastroenterology services for the term of September 1, 2021 through August 31, 2024, in an amount not to exceed $1,350,000.

BACKGROUND:
San Mateo County is responsible for providing necessary medical care to its medically indigent population. Dr. David Shields has provided gastroenterology services for San Mateo Medical Center since 2019 and at SMMC’s request, has increased the quantity of his services over that time period.
DISCUSSION:
Under this agreement, Dr. Shields has agreed to further increase his services in the area of call coverage, while maintaining a similar level of clinical services at a minimum of six half-day clinic blocks, during which he will see patients, consult, and perform procedures, including endoscopic retrograde cholangiopancreatography. Under this agreement, Dr. Shields will provide a minimum of 180 clinic blocks in endoscopy, including ERCP, to serve patient needs. His call coverage amounts are increasing from 88 weeknights to 96 per year, from 12 weekend calls to 16, and from 2 periods of holiday weekend call per year to 4.
County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.
The agreement and resolution have been reviewed and approved by County Counsel as to form. This agreement is coming to your Board late due to prolonged negotiations.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
It is anticipated that Dr. Shields’ patient documentation will be completed within 72 hours of seeing each patient, with all charts will be closed within 14 days thereafter, 100% of the time.

FISCAL IMPACT:
The term of the agreement is September 1, 2021 through August 31, 2024. The amount of the agreement is not to exceed $1,350,000 for the three-year term. Funds in the amount of $450,000 are included in the SMMC FY 2021-22 Approved Budget.
The payment provisions in this contract are based on the Median Gastroenterology 2020 Medical Group Management Association data, which SMMC uses to benchmark its contracted provider rates.
Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
