BACKGROUND:
After obtaining Board approval on June 27, 2017, by Resolution No. 075298, the County entered into an agreement with LifeMoves for the term of July 1, 2017 through June 30, 2018, for a total obligation amount not to exceed $712,000 to provide street-based outreach and community health services to assist San Mateo County residents who are unsheltered and experiencing homelessness with becoming housed and improve their access to health care.

After obtaining Board approval on June 5, 2018, by Resolution No. 075928, the County amended its agreement with LifeMoves to extend the term by two years for a new term of July 1, 2017 to June 30, 2020, and increased funding by $1,424,000 for a new total obligation amount not to exceed $2,136,000 to continue providing outreach and community health services for the expanded term.

After obtaining Board approval on June 2, 2020, by Resolution No. 077468, the County further amended its agreement with Life Moves to extend the term by an additional two years for a new term of July 1, 2017 to June 30, 2022, and increase funding by $1,098,524 for a new total obligation amount not to exceed $3,228,584 to continue providing outreach and community health services for the expanded term.
