Adopt a resolution authorizing an amendment to the agreement with LifeMoves, extending the term through December 31, 2020 and increasing the amount by $295,750, to an amount not to exceed $890,280.

BACKGROUND:
In September 2017, Healthcare for the Homeless/Farmworkers Health (HCH/FH) issued a Request for Proposals for providers to deliver enabling services to the homeless and farmworker populations of the County. The HCH/FH Program Co-Applicant Board selected Lifemoves for their innovative approach to providing such services. In February 2019, the Chief Executive Officer of San Mateo Medical Center (SMMC) approved an agreement with LifeMoves for enabling services.
DISCUSSION:
HCH/FH Board is requesting your Board approve an amendment to extend the term of the current agreement through December 31, 2020, with an associated increase in funding.
The agreement and resolution have been reviewed and approved by County Counsel as to form.
It is anticipated that a minimum of 650 individuals will receive care coordination and enabling services on an annual basis.

FISCAL IMPACT:
The term of the amended agreement is January 1, 2018 through December 31, 2020. The amount of the agreement is not to exceed $890,280. The amendment increases the contract amount by $295,750.
