BACKGROUND:

In September 2017, Healthcare for the Homeless/Farmworkers Health (HCH/FH) issued a Request for Proposals for providers to deliver enabling services to the homeless and farmworker populations of the County. The HCH/FH Program Co-Applicant Board selected Lifemoves for their innovative approach to providing such services. In February 2019, the Chief Executive Officer of San Mateo Medical Center (SMMC) approved an agreement with LifeMoves for enabling services.

 

DISCUSSION:

HCH/FH Board is requesting your Board approve an amendment to extend the term of the current agreement through December 31, 2020, with an associated increase in funding.

 

The agreement and resolution have been reviewed and approved by County Counsel as to form.

It is anticipated that a minimum of 650 individuals will receive care coordination and enabling services on an annual basis.

FISCAL IMPACT:

The term of the amended agreement is January 1, 2018 through December 31, 2020. The amount of the agreement is not to exceed $890,280. The amendment increases the contract amount by $295,750. Funds in the amount of $147,875 are included in the SMMC FY 2019-20 Adopted Budget. Funds in the amount of $147,875 will be included in the SMMC FY 2020-21 Recommended Budget.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC, and is within the existing annual appropriation.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
