BACKGROUND:

In June 2014, the Board approved an agreement with Optum Clinical Solutions, for ongoing support of the emergency department’s information system known as PulseCheck. The agreement has been amended multiple times and in April 2016, the agreement was reassigned to Picis Clinical Solutions, Inc.

DISCUSSION:

This amendment will add PulseCheck Injection and Infusion Services software application to capture charges for all medication infusions by assigning Common Procedural Terminology (CPT) codes. It is estimated that the San Mateo Medical Center (SMMC) coding department spends approximately 33 hours per month correcting claims with CPT errors. In addition, SMMC’s patient financial services department reports that over the past four months they have experienced a loss of approximately $34,000 in revenue ($101,000 on an annualized basis), as a result of CPT errors. The PulseCheck software is expected to minimize these CPT errors.

The amendment and resolution have been reviewed and approved by County Counsel as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

It is anticipated that there will be a 100% increase in revenue collection with improved CPT documentation.

FISCAL IMPACT:

The term of the amended agreement is July 1, 2014 through December 31, 2022. The amount of the agreement is not to exceed $960,000. This amendment increases the contract amount by $90,000. Funds in the amount of $90,000 are included in the SMMC FY 2019-20 Adopted Budget.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
