BACKGROUND:
The San Mateo County Health System has been awarded the Health Care for the Homeless/ Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services. The grant funds San Mateo Medical Center's (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC's continuum of care and other medical, health and support services. The HCH/FH Program has contracted with Puente de la Costa Sur (Puente) since 2013 to provide care coordination/ support services for farmworkers and their family members.

DISCUSSION:
In September 2017, HCH/FH issued a Request for Proposals (RFP) for providers to deliver health and health related services to the homeless and farmworker populations of San Mateo County. Upon review of the RFP submissions, the HCH/FH Program Co-Applicant Board selected Puente for its innovative approach in providing enabling and support and translation services to the County's farmworkers and their families. In addition, Puente will provide referral and care coordination with Sonrisas Dental Health Inc. for dental services in Pescadero.
