BACKGROUND:

The San Mateo County Health System has been awarded the Health Care for the Homeless/ Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services. The grant funds San Mateo Medical Center’s (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC’s continuum of care and other medical, health and support services. The HCH/FH Program has contracted with Puente de la Costa Sur (Puente) since 2013 to provide care coordination/ support services for farmworkers and their family members.

 

DISCUSSION:

In September 2017, HCH/FH issued a Request for Proposals (RFP) for providers to deliver health and health related services to the homeless and farmworker populations of San Mateo County. Upon review of the RFP submissions, the HCH/FH Program Co-Applicant Board selected Puente for its innovative approach in providing enabling and support and translation services to the County’s farmworkers and their families. In addition, Puente will provide referral and care coordination with Sonrisas Dental Health Inc. for dental services in Pescadero.

 

The agreement and resolution have been reviewed and approved by County Counsel as to form. This agreement is coming to your Board late due to protracted negotiations with the contractor.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

The agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing access to health care services to farmworker individuals of San Mateo County.  It is anticipated that a minimum of 200 unduplicated farmworker individuals will receive care coordination and other enabling services.

FISCAL IMPACT:

The term of the agreement is January 1, 2018 through December 31, 2018. The amount of the agreement is not to exceed $183,500 for its one-year term. Funds in the amount of $91,750 are included in the SMMC FY 2017-18 Adopted Budget. Funds in the amount of $91,750 will be included in the SMMC FY 2018-19 Recommended Budget.

 

The payment provisions are different from the previous agreement. There is a 55% increase in the amount from the previous agreement due to a 33% increase in the proposed number of individuals served, the addition of dental care coordination and translation services.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care are covered by the County’s General Fund contribution to SMMC, and are within the existing annual appropriation.
