Subject: Amendment to the Agreement with Sonrisas Dental Health Inc to Provide Dental Health Services
RECOMMENDATION:
title
Adopt a resolution authorizing an amendment to the agreement with Sonrisas Dental Health Inc to provide dental health services, extending the term by one year through December 31, 2024, and increasing the amount by $134,280 to an amount not to exceed $380,500.
body
BACKGROUND:
San Mateo County Health was awarded a Healthcare for the Homeless/Farmworkers Health (HCF/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration (HRSA) of the US Department of Health and Human Services. The HCH/FH grant funds San Mateo Medical Center's (SMMC) ongoing HCH/FH program, providing primary medical, dental, and behavioral health care to the homeless population.
In September 2021, Sonrisas Dental Health Inc. (Sonrisas) and SMMC agreed to provide dental health services to homeless and farmworker individuals in the County for the term of September 1, 2021, through April 30, 2022, in an amount not to exceed $65,000 as part of an HRSA grant. The agreement has been amended multiple times, most recently to extend the term through December 31, 2023, and increase the amount to the amount to at most $246,220.
DISCUSSION:
This amendment will enable Sonrisas to continue providing these much-needed dental services. Extending this agreement allows the services to continue through the HCH/FH Request for Proposals planning process for contracts to begin in January 2025. In addition, as this agreement is intended to provide dental services for farmworkers not covered by MediCal/DentaCal, and with MediCal coverage for immigrants ages 26 to 60 beginning in January 2024, this amendment will allow for continued services while SMMC determines the correct scope for future agreements.
County Attorney has reviewed and approved the resolution and amendment as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
The County’s Contract Compliance Committee has approved a waiver request for a forty-month term for this agreement.
It is anticipated that Sonrisas will provide 384 dental visits for homeless individuals, farmworkers, and their dependents presenting for dental health care services.

FISCAL IMPACT:
The term of the amended agreement is September 1, 2021, through December 31, 2024. The amendment increases the amount payable under the agreement by $134,280 to not exceeding $380,500 for the 40-month term. Funds in the amount of $67,140 are included in the SMMC FY 2023-24 Adopted Budget. Funds in the amount of $67,140 are included in the SMMC FY 2024-25 Recommended Budget.
Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
