Adopt a resolution authorizing an agreement with Darvin Scott Smith, MD, MS to provide infectious disease services for the term of January 1, 2024, through December 31, 2026, in an amount not to exceed $300,000.

BACKGROUND:

This agreement with Dr. Smith will permit him to provide on-call consultation services on a 24-hour shift, seven days per month. Dr. Smith’s specialty within infectious disease medicine is tropical diseases, and his availability to the County to provide regular consultation services to physicians and staff at San Mateo Medical Center (SMMC) enhances the County’s ability to detect and respond to emergent diseases within the community.

 

DISCUSSION:

The County independently contracts with several infectious disease physicians to establish and maintain the County’s capability of detecting and treating infectious diseases as they might emerge.

 

County Administrative Memorandum B-1 provides that physician contracts are exempt from the Request for Proposals process.

 

County Attorney has reviewed and approved the resolution and agreement as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

 

It is anticipated that Dr. Smith will timely respond to requests for consultation in accordance with SMMC’s on-call policy 95% of the time.

FISCAL IMPACT:

The term of the agreement is January 1, 2024, through December 31, 2026. The amount of the agreement is not to exceed $300,000 for the three-year term. Funds in the amount of $100,000 are included in the SMMC FY 2023-2024 Adopted Budget. Funds in the amount of $100,000 are included in the SMMC FY 2024-2025 Recommended Budget. Similar arrangements will be made for future years.

 

The payment provisions in the agreement are identical to those of the currently contracted infectious disease physicians at SMMC and do not represent any increase in the rate of physician compensation.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
