BACKGROUND:

In 2021 San Mateo Medical Center (SMMC) engaged Lucile Salter Packard Children’s Hospital (LCPH) to provide expert consultation services, which consisted of: having child abuse pediatricians provide care to abused minors; providing forensic evaluation services to the County to aid court proceedings; and acting as a liaison among SMMC medical and nursing professionals so that best practices in treating and evaluating child abuse may be followed.  The original agreement not to exceed the amount in 2021 was $110,000. 

 

In July 2022, SMMC and LCPH entered into a new agreement for the term of July 1, 2022, through June 30, 2023, in an amount not to exceed $189,332.

 

In February 2023, the parties amended the agreement to add on-call coverage services to the Keller Center with no change to the agreement amount and term.

 

DISCUSSION:

The current agreement with LPCH for child abuse pediatric evaluation and consultations at SMMC’s Keller Center was not previously brought to this Board because the fiscal impact originally fell below the $200,000 threshold for the Board of Supervisors’ approval under County Administrative Memo B-1. Likewise, because physician services are exempt from competitive bidding requirements under the same County memorandum, no informal solicitation or request for proposals were issued.

 

SMMC and LPCH are conducting negotiations to continue the pediatric child abuse consultation services and desire to extend the current agreement by one month such that the current amended agreement would expire on July 31, 2023.

Since the extension would result in an additional month’s worth of payments under the contract, approval is also being sought to increase the “not to exceed” amount by $25,000 to a limit of $214,332.

 

County Attorney has reviewed and approved the resolution and amendment as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

 

It is anticipated that 100% of LPCH physicians will be up to date regarding training for Keller Center examinations and protocols based on standards developed by the American Academy of Pediatrics and the American Professional Society on the Abuse of Children.

FISCAL IMPACT:

The term of the amended agreement is July 1, 2022, through July 31, 2023. The amount of the amended agreement is not to exceed $214,332 for the 13-month term. Funds in the amount of $214,332 are included in the SMMC FY 2022-2023 Adopted Budget.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
