BACKGROUND:

The cornerstone of the County’s Mental Health Services Act (MHSA) Community Services and Supports plan is to deliver Full Service Partnership (FSP) services for its highest risk consumers with serious mental illness. FSP programs are designed to allow consumers to remain living in the community and decrease their chances of becoming hospitalized or incarcerated by providing case management, medication management, therapy, social activities, and 24/7 response capability to mitigate an evolving crisis.

 

In November 2021, this Board approved an agreement with Caminar, Inc. for FSP services, for the term of July 1, 2021, through June 30, 2022, in an amount not to exceed $2,896,536.

 

In June 2022, this Board designated the Chief of San Mateo County Health or designee to approve a Cost-of-Living-Adjustment amendment and extend the term through June 30, 2023, increasing the amount by $3,159,831, to an amount not to exceed $6,056,367.

 

DISCUSSION:

Caminar provides 30 consumer slots for comprehensive FSP and Housing Support Program (HSP) services. FSP enrollees have services available 24 hours a day, 7 days a week. HSP services are provided to FSP enrollees to assist them in achieving independence, stability, and wellness. One of the strengths of Caminar’s FSP program is how it incorporates persons who have personal experiences in facing the challenges of mental illness and/or alcohol, as well as other drug addiction, as program staff. Through the Assisted Outpatient Treatment FSP, Caminar serves a target population of adults with serious mental illnesses that are unable to survive safely in the community without supervision, have a history of unsuccessful treatment experiences, and have a deteriorating ability to manage their symptoms.

 

Behavioral Health and Recovery Services (BHRS) will release an RFP for the FSP services in Winter 2023.  This nine month extension will provide the additional time for the RFP to be completed and allow Caminar to continue services throughout the proposed extension of the contract term.

 

The resolution contains the County’s standard provision allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

The amendment and resolution have been reviewed and approved by the County Attorney as to form.  

 

BHRS provides a range of services to promote wellness and recovery, and to support consumers remaining in the lowest possible level of care. FSP services for children, youth, and Transition Age Youth (TAY) is one established level of care. It is anticipated that 70% of the clients who receive FSP services will be maintained at a current or lower level of care.

FISCAL IMPACT:

The term of the amended agreement is July 1, 2021, through March 31, 2024. 

This amendment increases the agreement amount by $2,304,701 to an amount not to exceed $8,361,068 for the 33-month term. Of this increase, Mental Health Services Act will fund $1,637,596; Medi-Cal Federal Financial Participation will fund $468,233; 2011 Realignment (AB 109) will fund $57,430; Medicare will fund $15,629 and the remaining $125,813 will be funded by Net County Cost.
