BACKGROUND:

For over 15 years, Hope House, a program of Service League of San Mateo County (Service League), has provided an intensive residential treatment program for women who have a substance use disorder (SUD). Hope House services include safe and supportive transitional housing coupled with case management. Transitional housing residents must be employed and/or attend school, along with continued recovery-based involvement within the community.

 

In April 2016, the California Department of Health Care Services (DHCS) and the Federal Government approved the Drug Medi-Cal Organized Delivery System (DMC-ODS) waiver for substance use disorder treatment services. In March 2017, Service League began providing services under the DMC-ODS waiver, which allowed Service League and other providers to bill for many more services than they were previously able to.

 

In November 2021, DHCS continued to require changes in service delivery and reporting requirements of substance use disorder treatment services through the CalAIM waiver (formally DMC-ODS waiver). Due to these changes, BHRS requested this Board waive the Request for Proposals (RFP) process for substance use disorder treatment services for another year, allowing Behavioral Health and Recovery Services (BHRS) time to finalize the implementation of CalAIM waiver-related changes to this and all other BHRS substance use related contracts. As a result, BHRS contracted with Service League for the term of July 1, 2021, through June 30, 2022, in an amount not to exceed $895,990.

 

In June 2022, this Board designated the Chief of San Mateo County Health or designee to approve a Cost-of-Living-Adjustment amendment and extend the term through June 30, 2023, increasing the amount by $977,436, to an amount not to exceed $1,873,426. 

 

In December 2022, the Director of Behavioral Health and Recovery Services approved an amendment to the agreement to add a table of units of service, with no change to the amount or term of the agreement. 

 

DISCUSSION:

Through this amendment, Service League will continue to provide expanded residential substance use disorder treatment services for adult women, pregnant women, and women with infants; in addition, add Community Corrections Partnership (CCP) Sober Living Environment (SLE)/transitional housing services. 

 

BHRS will release an RFP for SUD services in Winter 2023. This extension will provide additional time for the RFP process to be completed and allow Service League to continue providing services throughout the proposed extension of the contract term. 

 

The resolution contains the County’s standard provision allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

The amendment and resolution have been reviewed and approved by the County Attorney as to form. 

 

Successful treatment discharge occurs when a program participant completes his/her treatment/recovery plan or when a participant is transferred to the next appropriate level of care. It is anticipated that 95% of program participants will achieve a successful treatment discharge.

FISCAL IMPACT:

The term of the amended agreement is July 1, 2021, through June 30, 2024. This amendment increases the agreement amount by $1,178,579 to an amount not to exceed $3,052,005 for the three-year term. Funds in the amount of $95,000 are included in the BHRS FY 2022-23 Adopted Budget, and $1,083,579 are included in the BHRS FY 2023-24 Recommended Budget. Of the FY 2023-24 amount, it is anticipated that the Mental Health Services Act will fund $22,084, Federal Financial Participation for Drug Med-Cal $309,151, Substance Abuse and Prevention Treatment Block Grant will fund $216,145, 2011 Realignment will fund $94,655, and the Net County Cost is $441,544.
