BACKGROUND:

For over 20 years, Behavioral Health, and Recovery Services (BHRS) has contracted with Our Common Ground, Inc. (OCG) to provide substance use disorder outpatient and residential treatment services to San Mateo County adults with substance abuse and addiction problems. 

 

In April 2016, the California Department of Health Care Services (DHCS) and the Federal Government approved the Drug Medi-Cal Organized Delivery System (DMC-ODS) waiver for substance use disorder treatment services.

 

In February 2017, OCG began providing services under the DMC-ODS waiver, which allowed OCG and other providers to bill for many more services than they were able to previously.

 

In November 2021, this Board approved an agreement with OCG to provide alcohol and other drug treatment services, for the term July 1, 2021, through June 30, 2022, in the amount of $3,758,357.

 

In September 2022, this Board approved an amendment to the agreement, extending the term through June 30, 2023, and increasing the amount by $4,099,992 to a new maximum of $7,858,349.

 

DISCUSSION:

Through this amendment, OCG will continue to provide substance use disorder outpatient and residential treatment services. Services include individual therapy, individual counseling, group therapy, anger management, family therapy, and room and board. OCG’s treatment approach is a strength-based Modified Therapeutic Community Model, which values relationships and views the clients in light of their competencies rather than their deficiencies. Success is based upon individual progress in treatment while concentrating on academic and vocational pursuits. 

 

BHRS will release an RFP for the selection of outpatient and residential substance use disorder treatment service providers in Fall 2023. This term extension will provide additional time for the RFP to be completed and allow OCG to continue providing services throughout the proposed contract term extension.

 

The resolution contains the County’s standard provisions allowing amendments to the County’s fiscal obligation by a maximum of $25,000 (in aggregate).

 

The amendment to the agreement and resolution have been reviewed and approved by County Attorney as to form.

 

Successful treatment discharge occurs when a program participant completes his/her treatment/recovery plan or when a participant is transferred to the next appropriate level of care. It is anticipated that 70% of program participants will achieve a successful treatment discharge.

FISCAL IMPACT:

The term of the amended agreement is July 1, 2021, through March 31, 2024. The amended maximum amount of the agreement of $7,858,349 is being increased in the amount of $2,990,431 to a new maximum of $10,848,779 for the 33-month term. The maximum for FY 2023-24 is $2,990,431, which is included in the BHRS FY 2023-24 Recommended Budget. Of that amount, Federal Financial Participation will fund $1,383,388 of the amendment; 2011 Realignment will fund $503,480; AB 109 will fund $276,056; Substance Abuse Block Grant will fund $230,540; Mental Health Services Act will fund $18,781, and the remaining $578,186 will be funded by Net County Cost.
