BACKGROUND:

Correctional Health Services (CHS) provides comprehensive physical health, mental health, substance use disorder treatment services, and nutrition consultation to incarcerated adults in San Mateo County. Due to several recent events, as described in the Discussion section below, CHS is seeking this Board’s approval for an Appropriation Transfer Request (ATR).

 

DISCUSSION:

n November 2021, through Coronavirus Emergency Supplemental Funding, administered by the Board of State and Community Corrections (BSCC), CHS received $1,193,398 from March 31, 2021, to January 30, 2023. A grant requirement of 20%, or $239,300, was allocated to non-governmental organizations that would support reentry backpacks consisting of personal protective equipment and personal hygiene kits, and motel vouchers for reentry quarantine and isolation purposes. Due to unsuccessful negotiations with vendors for motel vouchers and a grant time extension request, CHS could not expend $181,207. Additionally, $15,569 in salaries and benefits for staff was disallowed. As a result, per the BSCC grant agreement, CHS must return $196,776 to BSCC.

 

In March 2023, through the Epidemiology and Laboratory Capacity for Prevention and Control of Emerging Infectious Diseases, Detection, and Mitigation of COVID-19 funding program for confinement facilities, the California Department of Public Health (CDPH) awarded CHS a total of $309,156. The CDPH direct allocation letter is instead of a formal contract due to emergency funding, with funds supporting COVID-19 testing supplies and equipment. 

In April 2023, through the California Department of Health Care Services Medication Assisted Treatment (MAT) Expansion Project (MAT Expansion Project), as administered by Health Management Associates (HMA), CHS was awarded a total of $80,000 for participating in a MAT in jails and drug courts learning collaborative. HMA provided a Memorandum of Understanding for the MAT Expansion Project, with funds supporting a Narcan medication dispensing machine and related supplies in the Maguire Correctional Facility lobby and a sonogram machine.   

 

Finally, CHS drugs and pharmaceutical costs are projected to be over budget due to rising market costs, addressed in the FY 2023-25 budget planning. Other charges are also projected to be over budget due to an increase in General Liability insurance monthly rate from a recent claim.

 

FISCAL IMPACT:

Total expenditures needed at this time is $1,880,943 of which $1,723,654 is being covered by one-time Realignment funds. There is no change to Net County Cost as a result of this ATR.
