BACKGROUND:

The Juvenile Sexual Responsibility Program (JSR) serves a high-risk youth population with documented or suspected history of perpetrating sexual abuse. Members of this population are difficult to treat due to their need for close monitoring, specialized long-term treatment, and their high recidivism rate for sexually related offenses. Successful treatment of this population is dependent on the continuity of specialized mental health treatment and the development of close collaborative relationships between agencies treating, supervising, and monitoring the high-risk youth population.

 

In June 2019, this Board approved an agreement with the YMCA of San Francisco to provide JSR services and outpatient mental health services under the County Mental Health Managed Care Plan, in the amount of $1,213,494, for the term July 1, 2019, through June 30, 2022.

 

In September 2019, Behavioral Health and Recovery Services (BHRS) released a Request for Proposal for Trauma-Informed Co-Occurring Prevention Services for Youth. YMCA was one of four providers selected to provide these services to begin on July 1, 2020. Services provided would target youth and transitional age youth ages 15-25 who are at the greatest risk for adverse childhood experiences (ACE). Statistics have shown that nearly 50% of American children have experienced at least one ACE, with children of color and children who grow up in poverty showing the greatest risk for ACEs.

 

In December 2019, the Chief of San Mateo County Health (SMCH) approved an amendment to the agreement with the YMCA of San Francisco to provide a Cost-of-Living-Adjustment (COLA), increasing the agreement amount by $20,159 to an amount not to exceed $1,233,653, and with no change to the term of the agreement.

 

In January 2021, this Board approved an amendment to the agreement with the YMCA of San Francisco to terminate the provision of outpatient mental health services under the San Mateo County Mental Health Managed Care Plan, effective October 1, 2020, and to decrease the maximum amount by $300,000. At the same time, Trauma-Informed Co-Occurring Prevention services were added for FY 2020-21, increasing the maximum by $30,000 to an amount not to exceed $963,653, with no change to the term of the agreement.

 

In June 2022, this Board designated the Chief of SMCH or designee to approve a COLA amendment to the agreement to increase the amount by $122,566, to an amount not to exceed $1,146,219, and extending the agreement through June 30, 2023.

 

DISCUSSION:

BHRS is now requesting to amend its agreement with the YMCA of San Francisco for the provision of trauma-informed co-occurring prevention services for youth. This amendment will allow the YMCA of San Francisco to continue these services through June 2023.

The resolution contains the County’s standard provision allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

The amendment and resolution have been reviewed and approved by the County Attorney as to form.

It is anticipated that 93% of clients who receive JSR services through this amended agreement will not re-offend during the time between four months after admission and one year after completing the program.

FISCAL IMPACT:

The term of the amended agreement is July 1, 2019, through June 30, 2023. The amount of the amended agreement is not to exceed $1,146,219 for the three-year term. The amendment increases the contract maximum by $60,000 for BHRS FY 2021-23 for Trauma-Informed Co-Occurring Prevention services, which will be included in the BHRS FY 2021-23 Adopted Budget. It is anticipated that 100% or $60,000 will be funded by the Mental Health Services Act.
