BACKGROUND:

The Department of Health Care Services (DHCS) is implementing the California Advancing and Innovative Medi-Cal (CalAIM) initiative. CalAIM is designed as a long-term commitment to transform Medi-Cal, making the program more equitable, coordinated, and person-centered to help individuals maximize their health and life trajectory. CalAIM includes new benefits/services provided through Managed Care Plans for individuals requiring specialized care due to their complex needs. As part of this transformation, DHCS is providing funds to providers of Medi-Cal services to build capacity and infrastructure under the Capacity and Infrastructure Transition, Expansion, and Development initiative (CITED). This Board has previously approved two contracts between the Health Plan of San Mateo and County Health (HPSM) to provide similar services, specifically Enhanced Care Management (ECM) and Community Supports services.

 

DISCUSSION:

County Health intends to use these grant funds to build the capacity and infrastructure necessary to comply with CalAIM’s regulatory and programmatic demands. Specifically, these funds will be used to (1) provide training, to strengthen the skills and capacity of providers to deliver ECM services; (2) provide one-time funding to cover the costs of administrative staffing while various programs ramp up to capacity in preparation for CalAIM; (3) develop strategies to reach and engage hard to reach populations, including those experiencing homelessness, and; (4) develop workflows, technical specifications, and electronic applications necessary to increase care management capacity within an electronic health record, as well as to develop billing systems and data infrastructure necessary to submit claims and outcome data.

The resolution has been reviewed and approved by the County Attorney as to form.

This agreement is going to your Board late due to the delayed reward of grant funds provided with a retroactive start date from DHCS.

The number of persons referred to HPSM engaged in care is anticipated to increase by 30%.

FISCAL IMPACT:

The term of this agreement is from January 01, 2023, through December 31, 2024. The amount of this agreement is not to exceed $1,839,144 for the two-year term. This agreement is funded entirely through the CalAIM CITED grant. There is no Net County Cost associated with this agreement.
