BACKGROUND:

Dr. Trindade has been providing orthopedic services to patients at San Mateo Medical Center (SMMC) since August 2011 and has expressed interest in continuing to provide those services. During this time, Dr. Trindade has also provided medico-administrative services and leadership as SMMC’s Medical Director for Orthopedics.

 

DISCUSSION:

The County meets community need for orthopedic services through its contract with Dr. Trindade and other orthopedists.  Dr. Trindade will continue to provide the same level of services as stated in his previous agreement with the County.

 

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

 

County Attorney has reviewed and approved the resolution and agreement as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

It is anticipated that Dr. Trindade will arrive on time for each surgery and clinic scheduled 95% of the time.

FISCAL IMPACT:

The term of the agreement is January 1, 2023, through December 31, 2023. The amount of the agreement is not to exceed $468,800 for the one-year term. Funds in the amount of $273,466 are included in the SMMC FY 2022-23 Adopted Budget. Funds in the amount of $195,334 are included in the SMMC FY 2023-24 Recommended Budget. 

 

The total value of the new contract represents an 8.3% increase compared to that of the previous contract, which is due to an 8.3% increase in Dr. Trindade’s hourly compensation from $300.50 to $325.50. The higher rate of $ 325.50 is indicative of a blend of surveyed hourly compensation rates paid to orthopedists in the specialties of general orthopedic surgery and those orthopedists who specialize in “hip & joint” surgeries.  The blended rate for these two specialties most accurately captures the actual services being provided by Dr. Trindade.  The increased rates in the new contract are within fair market value according to VMG Health, a nationally recognized medical services valuation firm that relies upon multiple industry-standard provider compensation surveys and aggregates that data.    

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
