BACKGROUND:

Since 1985, this Board has approved resolutions authorizing expenditures for residential care facilities serving mentally ill clients and providing residential care services for County adults. These services enhance successful community living and avoid a more costly and higher level of care.

 

On June 23, 2020, this Board approved an agreement with Psynergy Programs, Inc. (Psynergy) for residential mental health services for the term of July 1, 2020, through June 30, 2021, in an amount not to exceed $1,692,694.

 

On June 29, 2021, this Board approved a first amendment to the agreement with Psynergy to extend the term through June 30, 2023 and increase the maximum amount by $3,158,649 to an amount not to exceed $4,851,343. 

 

DISCUSSION:

Psynergy provides a full range of mental health residential services, which includes: rehabilitative mental health services, psychiatric services, medication support, individual and group therapy, therapeutic programs with enhanced medical support for specific issues (e.g., diabetes), and a Modified Therapeutic Community. The programs emphasize independent living skills, nutrition, and exercise. Psynergy has facilities located in Morgan Hill, North Highlands, Sacramento, and Greenfield, California.

 

Behavioral Health and Recovery Services (BHRS) manages the placements in these and other residential service facilities within its total budgeted amount for mental health residential services.

 

BHRS is requesting to amend its current agreement to add Enhanced Support Services and COVID Isolation Support Services, consequently increasing the rates and increasing the maximum amount by $124,981 to an amount not to exceed $4,976,324.

 

Enhanced Support Services provide individual support and supervision for clients that need standby assistance and transportation for specialty medical appointments such as dialysis, physical therapy, and chemotherapy treatment.  COVID Isolation Support Services provide bedside support, medication delivery, and 1:1 support for clients ordered to isolate due to COVID. 

 

Under the B-1 Administrative Memorandum, the selection of providers of residential services for mental health clients is exempt from the Request for Proposals requirement.

The resolution contains the County’s standard provision allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

The agreement and resolution have been reviewed and approved by the County Attorney as to form.

 

It is anticipated that no more than 11% of all admissions will be discharged to an acute psychiatric level of care.

FISCAL IMPACT:

The term of the amended agreement is July 1, 2020, through June 30, 2023. The amount of the agreement is not to exceed $4,976,324 for the three-year term. Funds in the amount of $1,704,306 will be included in the BHRS FY 2022-23 Adopted Budget. It is expected that Federal Financial Participation will fund $378,888; 1991 Realignment will fund $1,224,720, and Net County Cost will fund $100,698.
