BACKGROUND:

Specialty plastic surgery services are provided under the general direction of the Chief Medical Officer and the Medical Director of Specialty Services at San Mateo Medical Center (SMMC).  Such care includes professional plastic surgery services, specifically in the area of non-cosmetic post traumatic, post-cancer, and hand-reconstructive care.

 

In September 2020, the County and Hubert Shih, MD entered into an agreement for plastic surgery services for the term of October 1, 2020, through September 30, 2022, in an amount not to exceed $600,000.

 

In July 2022, the parties amended the agreement to increase the contract amount by $25,000 to an amount not to exceed $625,000 with no change to the agreement term.

 

In September 2022, the parties amended the agreement to increase the contract amount by $25,000 to an amount not to exceed $650,000 and extend the agreement by one month through October 31, 2022.

 

DISCUSSION:

Under this amendment Dr. Shih has agreed to continue to provide plastic surgery services for one additional month, to allow for additional time to negotiate a new agreement.

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

 

The amendment and resolution have been reviewed and approved by the County Attorney as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

It is anticipated that 100% of plastic surgery services provided under this agreement will adhere to the infection control standards, as defined by the Joint Commission and Title XXII of the State of California, Code of Regulations, to reduce the risk of infections.

FISCAL IMPACT:

The term of the amended agreement is October 1, 2020, through November 30, 2022. The amendment increases the contract amount by $60,000 to an amount not to exceed $710,000 for the twenty-six-month term. Funds in the amount of $60,000 are included in the SMMC FY 2022-23 Adopted Budget.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
