Adopt a resolution authorizing the acceptance of Aging and Disability Resource Connection funding from the California Department of Aging in the amount of $245,448, and executing a provider agreement for the term of July 1, 2022 through June 30, 2023.

BACKGROUND:
Beginning in 2003, the Administration on Aging, now the Administration for Community Living, merged with the Centers for Medicare and Medicaid Services (CMS), to promote and fund Aging and Disability Resource Centers, which were designed to help individuals learn more about their community long-term services and supports (LTSS) options to promote the receipt of community-based LTSS in the most integrated setting possible.
Instead of LTSS, California has opted to use the modified title of Aging and Disability Resource Connection (ADRC) to reflect the principle of No Wrong Door (NWD), as opposed to a "single entry point" concept. This means that multiple agencies at the State and local level coordinate to create a simplified process through which individuals of all ages, abilities, and incomes can obtain unbiased information and one-on-one counseling on the options available in their communities. This allows individuals to make informed decisions about services that best meet their needs, regardless of where they enter the system. Consumers also receive assistance in accessing services, including help with applications for various public and private programs.
