BACKGROUND:

Data from 2013-2015 show that 40% of three-year-olds in the San Mateo County Head Start program had untreated dental decay. Children identified as Black/African American, Hispanic/Latino, Asian American and other minority race had higher levels of decay than White children. Many pregnant women and elderly lacked dental resources and services. The most affected regions of the County were: Daly City and Brisbane; central San Mateo; East Palo Alto, Redwood City and East Menlo Park; and coastal areas of Half Moon Bay, El Granada and Miramar.

 

To address these issues, the San Mateo County Oral Health Coalition began to work with consultants to develop a local needs assessment and strategic plan starting in FY 2015-16. The San Mateo County Oral Health Strategic Plan of 2017-2020, which included an amendment to also include 2021, has five main objectives: 1) to increase access to dental services and oral health education for children and pregnant women; 2) to increase access to dental services and oral health education for adults; 3) to build oral health provider capacity; 4) to increase awareness of the importance of oral health; and 5) to establish an oral health infrastructure and oral health surveillance system.

 

Funding for the Oral Public Health Program (OPHP), made available through the California Healthcare, Research and Prevention Tobacco Tax Act of 2016 (Prop 56), is to be used for oral public health services. The activities may include convening, coordinating, and collaborating to support planning, disease prevention, surveillance, education, and linkage to treatment programs.

 

On October 5, 2021, this Board approved a final amendment to a grant with the California Department of Public Health to provide funding in support of the OPHP, for the term January 1, 2018, through June 30, 2022, in an amount not to exceed $1,256,439.

 

DISCUSSION:

The purpose of the grant is to expand upon the OPHP by including the following program activities related to oral health in their communities: education, disease prevention, facilitating community-clinical linkages, and surveillance. This goal will be achieved by providing funding for activities that support demonstrated oral health needs and prioritize historically under-resourced populations, with a focus on population ages 0-5, and elementary school children and their parents/caregivers.

 

In order to minimize funding delays, Family Health Services (FHS) seeks this Board’s authorization for the Chief of San Mateo County Health or their designee to sign the grant, in lieu of this Board.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

 

The resolution has been reviewed and approved by the County Attorney as to form.

 

In FY 2020-21, 305 children received oral health screenings, and 805 received virtual oral health education due to COVID. It is anticipated that 1,500 children will receive oral health screenings and 1,200 will receive oral health education in FY 2021-22.

FISCAL IMPACT:

The term of the grant is July 1, 2022 through June 30, 2027. The amount of the grant is not to exceed $1,156,660 for the five-year term. Funds in the amount of $231,332 have been included in the FHS FY 2022-23 Recommended Budget. Similar arrangements will be made for future years. Since the County is receiving this funding, there is no Net County Cost.
