A) Adopt a resolution authorizing and directing the Chief of San Mateo County Health, or designee, to execute a fourth amendment to the grant award from the California Department of Public Health California Immunization Program for the term of July 1, 2017, through June 30, 2022, increasing the award amount by $2,802,955 to an amount not to exceed $7,427,798; and
B) Approve an Appropriation Transfer Request (ATR) in the amount of $1,875,292 appropriating unanticipated revenue to various expenditure accounts related to COVID-19 immunization expenses.

BACKGROUND:
The California Department of Public Health California Immunization Branch (CDPHIZB) provides funding to local health departments to ensure immunization program management, service delivery, population assessment, surveillance and outbreak control, provider quality assurance, consumer information, immunization data tracking, and vaccine management. CDPHIZB has funded the San Mateo County Immunization Program (SMCIP) for more than 20 years, providing outreach and education to the community and health care practitioners regarding the importance of complete and timely administration of vaccines as recommended by the Centers for Disease Control and Prevention.
On May 8, 2018, this Board approved an agreement with CDPHIZB to accept a grant award in support of immunization for the term of July 1, 2017, through June 30, 2022, in an amount not to exceed $1,398,560 (Round 1 funding).
On December 26, 2019, an amendment was approved by Family Health Services (FHS) to decrease the grant award by the amount of $50,349 due to federal budgetary constraints to an amount not to exceed $1,348,211 (Round 2 funding).
On May 4, 2021, this Board approved the second and third amendments, to: 1) extend the term of the grant through June 30, 2022 and increase the amount of the grant by $306,300; and 2) increase the amount of the grant by $2,970,332 for a total amount not to exceed $4,624,843 (Round 3 funding).
DISCUSSION:
SMCIP is located within FHS. The base CDPHIZB program (Round 1) supports general immunization-related outreach to elementary, middle, and high schools; childcare centers; training for pediatricians; reviews and assessments of Child Health and Disability Program providers; the perinatal hepatitis B prevention project; and participation in the California Automated Immunization Registry. Round 2 and Round 3 funding focused on general COVID-19 immunization efforts.
The purpose of the attached fourth grant amendment is to increase the award amount by $2,802,955 to an amount not to exceed $7,427,798. This additional money is intended for COVID-19 vaccination planning and implementation in FY 2021-22 with a focus on equity. The intent is to continue promotion and vaccine availability in communities that remain under reached. As currently planned, regular access to vaccine opportunities will remain static in those pockets of the community that trend with low vaccination/booster rates. Additionally, the focus will shift to support providers and community-based organizations to support the under 5 population once fully approved.
The amendment and resolution have been reviewed and approved by the County Attorney as to form.
These amendments have been created with the FHS director as the signatory. In order to minimize funding delays, FHS seeks this Board’s authorization for the Chief of San Mateo County Health, or designee, to sign the agreement, in lieu of this Board.
The Board is being asked to authorize the Chief of San Mateo County Health, or designee, to execute subsequent amendments which do not exceed an aggregate of $25,000.
It is anticipated that FHS’s focus and efforts in specific vaccination populations will continue to increase. In summary of FHS’s current performance measure, COVID-19 vaccines were not available until December 2020. After that, broad delivery did not start until January 2021. Through various eligibility criteria, FHS’s 12+ population became eligible in May 2021. The 5+ population did not become eligible until this fiscal year. While the data presented is accurate, the data is also a reflection of rolling eligibility criteria and will once again modify when the under 5 population become eligible. It is anticipated that 85% of the 5+ population across all ethnicity/race groups will become fully vaccinated within this fiscal year.

FISCAL IMPACT:
The term of the amended agreement is July 1, 2017, through June 30, 2022. This amendment increases the contract amount by $2,802,955 to an amount not to exceed $7,427,798 for the five-year term. Revenues for the base amount of this grant award through Round 3 funding have been included in the FHS FY 2021-22 Adopted Budget. The attached ATR in the amount of $1,875,292 accepts and distributes the amount of Immunization funding estimated to be spent in FY 2021-22. CDPHIZB has indicated that unspent funds will be rolled into the next year’s grant appropriation. Since the County is receiving this funding, there is no associated Net County Cost.
