BACKGROUND:
San Mateo County is responsible for providing necessary medical care to its medically indigent population. Since 2012 Dr. David Martin has provided urology services for San Mateo Medical Center (SMMC), treating disorders of the kidney, urethral tract and bladder. During this time, Dr. Martin also has served as SMMC’s Medical Director of Urology.
On September 28, 2021, your Board approved an agreement with Dr. David Martin to provide SMMC with urology services for the term of October 1, 2021 through September 30, 2024 in an amount not to exceed $2,150,000.
DISCUSSION:
Dr. Martin is one of two urologists providing services at SMMC. The other urologist Dr. Chris Threatt stopped providing services at SMMC effective March 1, 2022. Dr. Martin has agreed to increase his clinical commitment to cover Dr. Threatts’ services so that SMMC patient population does not experience a loss or delay in urology services. This amendment represents the increase in Dr. Martin’s annual clinical commitment from 192 to 240 clinic blocks and from 200 to 296 on-call days.
As a result, it is now necessary to increase the contract amount by $100,000, to an amount not to exceed $2,250,000, to cover the additional cost of urology services for the remainder of the agreement.
County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.
The agreement and resolution have been reviewed and approved by County Counsel as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
It is anticipated that 100% of Dr. Martin’s patients diagnosed with kidney stones will receive lithotripsy treatment within 24 hours of the diagnosis.

FISCAL IMPACT:
The term of the amended agreement is October 1, 2021 through September 30, 2024. The amendment increases the contract amount by $100,000 to an amount not to exceed $2,250,000 for the three year term. Funds in the amount of $497,544 are included in the SMMC FY 2022-23 Recommended Budget.
Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
