BACKGROUND:

San Mateo County is responsible for providing necessary medical care to its medically indigent population. Since August 1, 2011 Dr. Michael Trindade has provided orthopedic services for San Mateo Medical Center (SMMC), treating musculoskeletal issues.  During that time, Dr. Trindade also has served as SMMC’s Medical Director for Orthopedics.  

 

DISCUSSION:
Under this agreement, Dr. Trindade has agreed to provide a minimum of 96 half-day orthopedic clinics, including 24 fracture clinics, as well as a minimum of 120 half-day surgery blocks per year.

 

This agreement is going to your Board late due to prolonged negotiations with Dr. Trindade.

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

 

The agreement and resolution have been reviewed and approved by County Counsel as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

It is anticipated that 100% of joint surgery
wait times will be one month or less.

FISCAL IMPACT:

The term of the agreement is November 1, 2021 through October 31, 2022. The amount of the agreement is not to exceed $500,000 for the one-year term. Funds in the amount of $312,664 are included in the SMMC FY 2021-22 Adopted Budget.

 

The payment provisions in this contract are based on the 2020 data (released in 2021) for Orthopedic Surgery: Hip & Joint and Orthopedic Surgery: Trauma from Medical Group Management Association, which SMMC uses to benchmark its contracted provider compensation and productivity. 

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
