BACKGROUND:
In 2014, healthRIGHT 360 (HR360) merged with two community-based organizations that independently contracted with the County: Asian American Recovery Services and Women’s Recovery Association. HR360 services include substance use prevention and treatment services, and the North County Outreach Collaborative (NCOC), which provides outreach, linkage, and case management services to the emotionally disturbed population residing in the northern region of the County.
In April 2016, the California Department of Health Care Services (DHCS) and the Federal Government approved the Drug Medi-Cal Organized Delivery System (DMC-ODS) waiver for substance use disorder treatment services. In March 2017, HR360 began providing services under the DMC-ODS waiver, which allowed HR360 and other providers to bill for many more services than they were previously able.
DISCUSSION:
Through the DMC-ODS waiver, HR360 continues to provide substance use disorder treatment services to adults, youth, and families including: outpatient and residential treatment, medication assisted treatment, sober living environments, and case management services. HR360 will also serve as the fiscal and lead agency for the NCOC by providing culturally appropriate outreach, psycho-education, screening, referral and warm hand-off services. In addition, HR360 shall provide mental health outreach coupled with vaccine outreach to enhance vaccine equity in East Palo Alto. Lastly, HR360 shall continue to be the fiscal and lead agency for the North County Prevention Partnership by providing substance use prevention services in South San Francisco, San Bruno, Brisbane, and Millbrae.
DHCS continues to require changes in service delivery and reporting requirements of substance use disorder treatment services through the DMC-ODS waiver. Due to these changes, Behavioral Health and Recovery Services (BHRS) is requesting that your Board waive the Request for Proposals (RFP) process for substance use disorder treatment services for one year, allowing BHRS time to finalize the implementation of DMC-ODS waiver-related changes to this and all other BHRS substance use related contracts. In 2021, BHRS will release an RFP for substance use disorder treatment services to start on July 1, 2022.
The resolution contains the County’s standard provision allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).
The agreement and resolution have been reviewed and approved by County Counsel as to form. This agreement has been delayed due to prolonged contract negotiations and is on the Continuing Resolution.
Successful treatment discharge occurs when a program participant completes his/her treatment/recovery plan or when a participant is transferred to the next appropriate level of care. It is anticipated that 58% of program participants will achieve a successful treatment discharge.

FISCAL IMPACT:
The term of the agreement is July 1, 2021 through June 30, 2022. The amount of the agreement is not to exceed $4,356,068 for the one-year term and is included in the BHRS FY 2021-22 Adopted Budget. Measure K will fund $345,347; Mental Health Services Act will fund $425,024; Federal Financial Participation for Drug Med-Cal will fund $1,881,574; Substance Abuse and Prevention Treatment Block Grant will fund $329,020; 2011 Realignment will fund $191,261; and AB109 will fund $115,283. The remaining $1,068,559 is Net County Cost.
