BACKGROUND:

San Mateo County is responsible for providing necessary medical care to its medically indigent population. Dr. Bryan Gescuk has provided rheumatology services for San Mateo Medical Center (SMMC) for the past 17 years and is currently the only   rheumatologist providing services at SMMC.

 

DISCUSSION:
Under this agreement, Dr. Gescuk would continue to provide rheumatology services at SMMC, consisting of at least four and up to seven half-day clinic blocks per week, as well as call coverage. Additionally, Dr. Gescuk will be providing from four to eight hours of administrative services each week, depending upon need, including training and leading providers in the Provider Engagement Group. Dr. Gescuk is well positioned to perform these services, given his familiarity and experience with SMMC’s patient population and County administrative functions through his tenure with SMMC.

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

 

The agreement and resolution have been reviewed and approved by County Counsel as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

It is anticipated that Dr. Gescuk’s patient documentation will be completed within 72 hours of seeing each patient, with all charts closed within 14 days thereafter, 100% of the time.

FISCAL IMPACT:

The term of the agreement is October 1, 2021 through September 30, 2024. The amount of the agreement is not to exceed $1,225,000 for the three-year term. Funds in the amount of $400,000 are included in the SMMC FY 2021-22 Approved Budget.

 

The payment provisions in this contract are based on the 2020 data (released in 2021) for Rheumatology from Medical Group Management Association, which SMMC uses to benchmark its contracted provider compensation and productivity. 

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
