BACKGROUND:

On July 23, 2019, the Board approved an agreement with HealthRIGHT360 (HR360) to provide substance use disorder prevention services for the term July 1, 2019 through June 30, 2020, and substance use disorder treatment services, medication-assisted treatment services, outpatient mental health services, and outreach and engagement services, for the term July 1, 2019 through June 30, 2021, in an amount not to exceed $9,993,072.

 

On September 24, 2019, the Board approved the FY 2019-20 Adopted Budget.  Previously, on June 25, 2019, the Board received the FY 2020-21 Preliminary Recommended Budget. These budgets included a 4% Cost of Living Adjustment (COLA) for Behavioral Health and Recovery Services (BHRS) community-based organizations that provide mental health services and substance abuse disorder services.

 

On December 3, 2019, the Board approved an amendment to the agreement to: 1) add the COLA to substance use disorder prevention and treatment services, mental health services, and outreach and engagement services; and 2) remove the Parent Project curriculum, increasing the amount by $543,261 to an amount not to exceed $10,536,333, with no change to the term of the agreement.

 

On April 21, 2020, the Board approved an amendment to the agreement to adjust prevention activities in FY 2019-20 and add prevention activities in FY 2020-21, extending the term of the agreement through June 30, 2021, and increasing the amount of the agreement by $143,412 to and amount not to exceed $10,679,744.

 

On October 6, 2020, the Chief of San Mateo County Health approved an amendment to the agreement to terminate mild to moderate mental health services authorized by the Mental Health Plan effective September 30, 2020, with no change to the maximum amount or term of the agreement.

 

On April 6, 2021, the Board approved an amendment to the agreement to close the HR360 East Palo Alto clinic effective December 31, 2020, decreasing the amount of the agreement by $243,608 to an amount not to exceed $10,436,136, with no change to the agreement term.

 

DISCUSSION:

HR360 currently provides: 1) substance use disorder treatment services to adults, youth, and families including outpatient and residential treatment; 2) medication-assisted treatment; 3) sober living environments; and 4) case management services. Additionally, HR360 serves as the fiscal and lead agency for the North County Outreach and Collaborative by providing culturally appropriate outreach, psychoeducation, screening, referral, and warm hand-off services. Lastly, HR360 serves as the fiscal and lead agency for the North County Prevention Partnership by providing substance use prevention services in Daly City and Pacifica.

 

Through this amendment, HR360 shall provide ASAM 3.3 residential services. ASAM 3.3 is a 24-hour clinically managed high intensity residential service with trained counselors, serving individuals that are in multi-dimensional imminent danger. Less intense milieu and group treatment is available for those with cognitive or other impairments that are unable to utilize the full active milieu or therapeutic community in preparation for outpatient treatment. HR360 is the only contractor in San Mateo County’s network of care that provides these necessary 3.3 services. In addition, HR360 shall provide mental health outreach coupled with vaccine outreach to enhance vaccine equity in East Palo Alto. As a result, it is now necessary to amend this agreement to increase the maximum amount of the agreement by $130,000 to an amount not to exceed $10,566,136.

 

The resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

The amendment and resolution have been reviewed and approved by County Counsel as to form. 

 

It is anticipated that 58% of clients will achieve a successful treatment discharge.

FISCAL IMPACT:

The term of the amended agreement is July 1, 2019 through June 30, 2021. The amount of the amended agreement is not to exceed $10,566,136 for the two-year term. The amendment increases the contract amount by $130,000 and is included in the BHRS FY 2020-21 Adopted Budget. Of the increase, $72,960 will be funded by Drug Medi-Cal Organized Delivery Service, $23,040 will be funded by 2011 Realignment, $24,000 will be funded by Substance Abuse Block Grant and $10,000 will be funded by one-time Mental Health Services Act, COVID-19 client support program.
