Adopt a resolution authorizing an agreement with Tarunpreet Bains, MD to provide hematology oncology services for the term of August 1, 2021 through July 31, 2024, in an amount not to exceed $1,290,000.

BACKGROUND:
San Mateo County is responsible for providing necessary medical care to its medically indigent population, as well as access to services to its general population. Dr. Bains has been providing hematology oncology services at San Mateo Medical Center (SMMC) since 2017.
DISCUSSION:
The County’s need for hematology oncology services remains substantial due to the medical needs of its patient population. Dr. Bains has agreed to provide 0.7 FTE of hematology oncology services, the same level of services she provided in the previously amended agreement and will continue to share call coverage with the other hematology oncology provider under contract with SMMC.
County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.
The agreement and resolution have been reviewed and approved by County Counsel as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
It is anticipated that the percentage of outside referrals made for hematology oncology services will be reduced by 15%.

FISCAL IMPACT:
The term of the agreement is August 1, 2021 through July 31, 2024. The amount of the agreement is not to exceed $1,290,000 for the three-year term. Funds in the amount of $430,000 are included in the SMMC FY 2021-22 Recommended Budget. Similar arrangements will be made for future years of the term.
The contract’s not-to-exceed amount is consistent with the amount of the prior agreement, which was for two years. The payment provisions of this contract are based on the Median Hematology Oncology 2020 Medical Group Management Association Provider Compensation and Production Report, which SMMC uses to benchmark its contracted provider rates.
Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
