BACKGROUND:

San Mateo County is responsible for providing necessary medical care to its medically indigent population, as well as access to services to its general population. Dr. Buckley has been providing ophthalmology services at San Mateo Medical Center (SMMC) for more than 20 years.

 

DISCUSSION:

The County’s need for ophthalmology services remains substantial due to the aging of its patient population. Dr. Buckley will be providing the same level of ophthalmology services and the same call coverage as his previous agreement: four clinic blocks and four surgery blocks per month. He will continue to share call coverage with the other two ophthalmologists under contract with SMMC.

 

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

 

The agreement and resolution have been reviewed and approved by County Counsel as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).  This contract is late due to prolonged negotiations.

It is anticipated that 85% of diabetic patients will receive annual retinal/dilated eye exam.

FISCAL IMPACT:

The term of the agreement is July 1, 2021 through June 30, 2024. The amount of the agreement is not to exceed $460,000 for the three-year term. Funds in the amount of $153,333 are included in the SMMC FY 2020-21 Recommended Budget. Similar arrangements will be made for future years.

 

There is no increase in the payment provisions of this agreement compared to the previous agreement. The payment provisions are based on the Median Ophthalmology 2019 Medical Group Management Association Provider Compensation and Production Report, which SMMC uses to benchmark its contracted provider rates.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
