BACKGROUND:
San Mateo County Health has been awarded the Health Care for the
Homeless/Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care,
Health Services and Resources Administration of the US Department of Health and
Human Services. The grant funds San Mateo Medical Center’s (SMMC) ongoing
HCH/FH program, which is designed to assist homeless and farmworker adults,
families, and youth with access to and utilization of SMMC’s continuum of care and
other medical, health and support services.
DISCUSSION:
In January 2021, the HCH/FH program completed a Request for Proposals (RFP) for enabling services, which are non-clinical services that enable individuals to access health care and improve health outcomes. Enabling services include case management, referrals, translation/interpretation, transportation, eligibility assistance, health education, environmental health risk reduction, health literacy, and outreach. LifeMoves was chosen to provide these services, based on their efficiency in delivering enabling services to the population they serve.
County Counsel has reviewed and approved the resolution and agreement as to form.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
The County’s Contract Compliance Committee has approved a waiver request for a forty-two-months term for this agreement.
It is anticipated that a minimum of 400 unduplicated individuals experiencing homelessness annually will receive care coordination or assistance with applying for or renewing their health insurance coverage.

FISCAL IMPACT:
The term of the agreement is July 1, 2021 through December 31, 2024. The amount of the agreement is not to exceed $1,216,500 for the forty-two-months term. Funds in the amount of $347,571 are included in the SMMC FY 2021-22 Adopted Budget. Similar arrangements will be made for future years.
The payment provisions and levels of services in this instant agreement differ from those in the previous agreement between the County and LifeMoves. The total amount authorized to be expended under this new agreement is 16% more than the amount authorized under the prior agreement. This is due to a shift in service scope that emphasizes ensuring that clients establish a medical home, are able to schedule and attend appointments at SMMC outpatient clinics with LifeMoves’ assistance and adhere a medical care plan in collaboration with their primary care physician. Providing these services is anticipated to be more expensive than providing the services called for under the prior agreement.
Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC and is within the existing annual appropriation.
