BACKGROUND:
San Mateo County Health has been awarded the Health Care for the Homeless/Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services. The grant funds San Mateo Medical Center's (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC's continuum of care and other medical, health and support services.

DISCUSSION:
In January 2021, the HCH/FH program completed a Request for Proposals (RFP) for enabling services. Enabling services are non-clinical services that do not include direct patient services, which enable individuals to access health care and improve health outcomes. Examples of enabling services include: case management, referrals, translation/interpretation, transportation, eligibility assistance, health education, environmental health risk reduction, health literacy, and outreach. Puente de la Costa Sur (Puente) was chosen for their efficiency in delivering enabling services to the population they serve.

County Counsel has reviewed and approved the resolution and agreement as to form.

The resolution contains the County's standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
