A) Adopt a resolution authorizing and directing the Chief of San Mateo County Health, or designee, to execute 1) a second amendment to the grant award from the California Department of Public Health California Immunization Program for the term of July 1, 2017 through June 30, 2022, increasing the award amount by $306,300 to an amount not to exceed $1,654,511 and 2) a third amendment increasing the award amount by $2,970,332 to an amount not to exceed $4,624,843 as noted in a Letter of Intent from the California Department of Public Health; and
B) Approve an Appropriation Transfer Request (ATR) in the amount of $1,090,266 appropriating unanticipated revenue to various expenditure accounts related to influenza and COVID-19 immunization expenses.

BACKGROUND:
The California Department of Public Health California Immunization Branch (CDPHIZB) provides funding to local health departments to ensure immunization program management, service delivery, population assessment, surveillance and outbreak control, provider quality assurance, consumer information, immunization data tracking, and vaccine management. CDPHIZB has funded the San Mateo County Immunization Program (SMCIP) for more than 20 years, providing outreach and education to the community and health care practitioners regarding the importance of complete and timely administration of vaccines as recommended by the Centers for Disease Control and Prevention.
On May 8, 2018, your Board approved an agreement with CDPHIZB to accept a grant award in support of immunization for the term of July 1, 2017 through June 30, 2022, in an amount not to exceed $1,398,560.
On December 26, 2019 an amendment was approved by Family Health Services (FHS) to decrease the grant award by the amount of $50,349 due to federal budgetary constraints to an amount not to exceed $1,348,211.
DISCUSSION:
SMCIP is located within FHS. The base CDPHIZB program supports immunization-related outreach to elementary, middle, and high schools; childcare centers; training for pediatricians; reviews and assessments of Child Health and Disability Program providers; the perinatal hepatitis B prevention project; and participation in the California Automated Immunization Registry.
The purpose of the attached second grant amendment increases funding to augment and enhance efforts around influenza vaccination coverage, as well as to perform COVID-19 vaccination planning and implementation in FY 2020-21.
Execution of the second amendment is necessary before the County may access funds promised in a Letter of Intent (LOI) from CDPH in the form of a third amendment to this agreement. The LOI documents increasing the award amount by $2,970,332 to an amount not to exceed $4,624,843. This additional money in the third amendment is intended for COVID-19 vaccination planning and implementation in FY 2020-21 and FY 21-22. In order to assure that funding is available during FY 2020-21 CDPHIZB requires an executed agreement by June 4, 2021. Therefore, in order to expedite processing, we are requesting that your Board approve execution of the third amendment as proposed in the LOI. This will allow the Chief of County Health or designee to execute the third amendment upon release.
The amendment and resolution have been reviewed and approved by County Counsel as to form.
These amendments have been created with the FHS Director as the signatory. In order to minimize funding delays, FHS seeks your Board’s authorization for the Chief of San Mateo County Health, or designee, to sign the agreement, in lieu of your Board.
The Board is being asked to authorize the Chief of San Mateo County Health, or designee, to execute subsequent amendments which do not exceed an aggregate of $25,000.
It is anticipated that 7,400 flu vaccines will be administered and 600,000 people will receive a full series of COVID-19 vaccinations.

FISCAL IMPACT:
The term of the agreement is July 1, 2017 through June 30, 2022. The amount of the agreement including both the second and third amendments is not to exceed $4,624,843 for the five-year term. Revenues and expenditures for the base amount of this grant award ($279,712) have been included in the FHS FY 2020-21 Adopted Budget.
The attached ATR appropriates unanticipated revenue and expenses for $1,090,266, which represents the portion of increased funding that is expected to be expended in FY 20-21. The balance of the COVID-19 funding will be appropriated in FY 21-22.
