BACKGROUND:

On December 4, 2018, the Board approved an agreement with Jefferson Union High School District (JUHSD) to provide mental health services and alcohol and other drug (AOD) prevention services for the term of July 1, 2018 through June 30, 2020, in an amount not to exceed $655,180.

 

On July 21, 2020, your Board approved an amendment to the agreement for alcohol and other drug prevention services and mental health services, extending the term through June 30, 2021, and increasing the amount by $404,543, to an amount not to exceed $1,059,723.

 

On October 1, 2020, the Chief of San Mateo County Health approved an amendment to the agreement to terminate the mild to moderate mental health services authorized by the Mental Health Plan. Behavioral Health and Recovery Services (BHRS) worked with the Health Plan of San Mateo (HPSM) to transition the administrative functions of the mild to moderate mental health services from BHRS to HPSM. The completion date of the transition and termination of the services within this agreement, was September 30, 2020.

 

On January 25, 2021, the Chief of San Mateo County Health approved an amendment to the agreement to add technology supports for clients and family members that do not have the resources to purchase the technology they need to participate in telehealth services and online groups, increasing the amount by $4,437 to an amount not to exceed $1,064,160, with no change to the agreement term.

 

DISCUSSION:

JUHSD operates the Daly City Youth Health Center (DCYHC) which provides youth: 1) outpatient mental health services in Pacifica, Daly City, Colma, Brisbane and parts of South San Francisco; and 2) AOD prevention services to reduce access to and availability of AOD to youth. 

 

During the COVID-19 health crisis, the DCYHC has responded by implementing the Allminds telehealth platform, geared specifically toward mental health services, and incorporates pre and post questions to clients in order to measure wellbeing and service satisfaction. Once trained, clinicians: 1) created therapeutic ways to adapt to pandemic obstacles; 2) implemented well check questions and increased case management in order to address an increase in basic needs due to the pandemic (job loss, health issues, food shortage); and 3) made arrangements to safely meet clients in person to help them learn how to use technology to access telehealth and school online, and provided headphones to clients who needed them for privacy. The DCYHC ordered more laptops and VPN access for clinicians to access Avatar remotely and created a new screening and referral process to compensate for not having in-person warm hand-offs.

 

This year there has been an increase in referrals of clients aged 21-25. It is now necessary to amend the agreement to meet the need for additional outpatient mental health services, increasing the amount of the agreement by $75,000. Beginning July 1, 2021, a new contract will be created for this population, in an effort to meet the continued need for services.

 

The resolution contains the County’s standard provision allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

The amendment and resolution have been reviewed and approved by County Counsel as to form.

 

BHRS provides a range of services to promote wellness and recovery and to support consumers remaining in the lowest possible level of care. The provision of outpatient mental health services is one established level of care. It is anticipated that 99% of clients who receive services through this agreement with JUHSD will be maintained at a current or lower level of care.

FISCAL IMPACT:

The term of the amended agreement is July 1, 2018 through June 30, 2021. The amount of the amended agreement is not to exceed $1,139,160. The amendment increases the contract maximum by $75,000 and is included in the BHRS 2020-21 Adopted Budget. Of the increase, Medi-Cal Federal Financial Participation will fund $37,500; 2011 Realignment will find $37,500. There is no Net County Cost.
