BACKGROUND:

On July 23, 2019, the Board approved an agreement with HealthRIGHT360 (HR360) to provide substance use disorder prevention services for the term July 1, 2019 through June 30, 2020, and substance use disorder treatment services, medication assisted treatment services, outpatient mental health services, and outreach and engagement services, for the term July 1, 2019 through June 30, 2021, in an amount not to exceed $9,993,072.

 

On September 24, 2019, the Board approved the FY 2019-20 Adopted Budget.  Previously, on June 25, 2019, the Board received the FY 2020-21 Preliminary Recommended Budget. These budgets included a 4% Cost of Living Adjustment (COLA) for Behavioral Health and Recovery Services (BHRS) community-based organizations that provide mental health services and substance abuse disorder services.

 

On December 3, 2019, the Board approved an amendment to the agreement to: 1) add the COLA to substance use disorder prevention and treatment services, mental health services, and outreach and engagement services; and 2) remove the Parent Project curriculum, increasing the amount by $543,261 to an amount not to exceed $10,536,333, with no change to the term of the agreement.

 

On April 21, 2020, the Board approved an amendment to the agreement to adjust prevention activities in FY 2019-20, add prevention activities in FY 2020-21 extending the term of the agreement through June 30, 2021, and increasing the amount of the agreement by $143,412 to $10,679,744.

 

On October 6, 2020, the Chief of San Mateo County Health approved an amendment to the agreement to terminate mild to moderate mental health services authorized by the Mental Health Plan effective September 30, 2020, with no change to the maximum amount or term of the agreement.

 

DISCUSSION:

HR360 currently provides: (1) substance use disorder treatment services to adults, youth, and families including outpatient and residential treatment; (2) medication assisted treatment; (3) sober living environments; and (4) case management services. Additionally, HR360 serves as the fiscal and lead agency for the North County Outreach and Collaborative by providing culturally appropriate outreach, psycho-education, screening, referral, and warm hand-off services. Lastly, HR360 serves as the fiscal and lead agency for the North County Prevention Partnership by providing substance use prevention services in Daly City and Pacifica.

 

Since 2018, HR360’s East Palo Alto (EPA) clinic has delivered individual and group therapy, crisis intervention, and case management services to underserved youth in the community. In the last few years, changes to court ordered substance youth treatment and other factors have resulted in fewer referrals and an under-utilization of services.  BHRS and HR360 mutually agreed to close the EPA clinic on December 31, 2020.  As a result of the closure, it is now necessary to amend this agreement to reduce the maximum amount of the agreement by $243,608.

 

The resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

The amendment and resolution have been reviewed and approved by County Counsel as to form. 

 

It is anticipated that 58% of clients will achieve a successful treatment discharge.

FISCAL IMPACT:

The term of the amended agreement is July 1, 2019 through June 30, 2021. The amount of the amended agreement is not to exceed $10,436,136. The amendment decreases the contract amount by $243,608 and is included in the BHRS FY 2020-21 Adopted Budget. Of the decrease, 2011 Realignment funding will be reduced by $243,608.
