Adopt a resolution authorizing an amendment to the agreement with LifeMoves to provide enabling services, extending the term of the agreement through June 30, 2021 and increasing the amount payable by $154,900, to an amount not exceed $1,045,180.

BACKGROUND:
In September 2017, the Health Care for the Homeless / Farmworker Health Program (HCH/FH) completed a Request for Proposals (RFP) for enabling services. LifeMoves was chosen to provide enabling services, which are non-clinical services that enable individuals to access health care and improve health outcomes. Enabling services include case management, referrals, translation/interpretation, transportation, eligibility assistance, health education, environmental health risk reduction, health literacy, and outreach.
The agreement has been amended previously; most recently, your Board approved an amendment extending the term of the agreement through December 31, 2020, in an amount not to exceed $890,280. This agreement is funded through a grant received from the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA).
DISCUSSION:
HCH/FH had planned to release an RFP for enabling services in the second quarter of 2020, in order to have new agreements in place by January 1, 2021. However, due to the COVID-19 pandemic and the need to continue providing services to individuals experiencing homelessness, this was not feasible.
