BACKGROUND:

In September 2017, the Health Care for the Homeless / Farmworker Health Program (HCH/FH) completed a Request for Proposals (RFP) for enabling services. Enabling services are non-clinical services that do not include direct patient services, which enable individuals to access health care and improve health outcomes. Examples of enabling services include: case management, referrals, translation/interpretation, transportation, eligibility assistance, health education, environmental health risk reduction, health literacy, and outreach.

 

Puente de la Costa Sur was chosen for their efficiency in delivering enabling services to the population they serve. The agreement has been amended previously, most recently by your Board on April 9, 2019, increasing the amount by $367,000 to an amount not to exceed $550,500. This agreement is funded through a grant received from the U.S. Department of Health and Human Services, Health Resources, and Services Administration (HRSA).

 

DISCUSSION:

HCH/FH had planned to release an RFP for enabling services in the second quarter of 2020, in order to have new agreements in place for enabling services by January 1, 2021. However, due to the COVID-19 pandemic and the continuing need to provide these services to the farmworker community, this was not possible. Therefore, San Mateo Medical Center (SMMC) proposes that your Board approve an amendment extending the term of the agreement through June 30, 2021 and increasing the amount by $123,250, to an amount not to exceed $673,750.

The agreement and resolution have been reviewed and approved by County Counsel as to form.

 

The resolution contains the County’s standard provisions allowing further amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

The County’s Contract Compliance Committee has approved a waiver request for a forty-two month term for this agreement.

 

It is anticipated that an additional 245 unduplicated farmworker individuals will receive enabling services through June 30, 2021.

FISCAL IMPACT:

The term of the amended agreement is January 1, 2018 through June 30, 2021. The amount of the agreement is not to exceed $673,750. This amendment increases the contract amount by $123,250. Funds in the amount of $123,250 are included in the SMMC FY 2020-21 Adopted Budget.

 

Expenses for this agreement are completely funded by the ongoing Section 330 Public Health Act Community Health Center grant award from HRSA.
