BACKGROUND:

In 2018, Behavioral Health and Recovery Services (BHRS) identified that there was a need to fill a service gap for severely mentally ill youth, adolescent, and adult clients who are diagnosed with moderate to severe eating disorders, as defined in the DSM-5, who require a more specialized and intensive level of treatment. At the time, no BHRS clinic or program effectively addressed the comprehensive behavioral, dietary, and medical needs associated with moderate to severe eating disorders. BHRS reached out to several local providers and Center for Discovery (CFD) was one of two providers that met the requirements to provide comprehensive and specialized treatment for clients diagnosed with eating disorders such as: anorexia, bulimia, binge eating disorder, and eating disorders not otherwise specified. 

In July 2018, BHRS conducted an open and fair process to select a provider through obtaining three quotes. CFD submitted a quote and BHRS entered into an agreement with CFD to provide residential treatment to youth, adolescent, and adult clients in the county for the term of June 1, 2018 through June 30, 2020. Residential treatment services are exempt from the Request for Proposal process under the B-1 Administrative Memorandum.

 

DISCUSSION:

CFD services range from nutritional assessments, comprehensive treatment plans, close supervision, dietary counseling and individual and family counseling. Due to identification of a continued need for eating disorder services, BHRS is requesting to enter into a new agreement with CFD. 

 

During the COVID-19 health crisis, CFD has been maintaining service expectations by implementing extensive policies and protocols to manage COVID-19 risk including staff and patient screening, staff and patient testing, isolation or quarantine in the event of exposure, suspected cases, or confirmed cases. They have also implemented protocols for more frequent cleaning as well as cleaning protocols for when a COVID-19 positive individual enters any of their sites. They also have plans for staffing coverage in the event multiple staff members are out sick with COVID-19.

 

The resolution contains the County’s standard provision allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

The agreement and resolution have been reviewed and approved by County Counsel as to form. This agreement is late due to prolonged contract negotiations and is on the Continuing Resolution.

 

It is anticipated that 75% of clients completing the program will be successfully served at a lower level of care for up to one year, after being discharged.

FISCAL IMPACT:

The term of the agreement is July 1, 2020 through June 30, 2022. The amount of the agreement is not to exceed $500,000 for the length of the term. Of this amount, it is anticipated that $250,000 will be funded for FY 2020-21. It is anticipated that 100% of the contract will be funded by Mental Health Services Act. Funds in the amount of $250,000 are included in the BHRS FY 2020-21 Recommended Budget. Similar arrangements will be in place for FY 2021-22. There is no Net County Cost associated with this agreement.
