BACKGROUND:

The California Department of Public Health (CDPH) provides funding for Tuberculosis (TB) control activities including the investigation of potential cases and administration of the directly-observed therapy program. The increased TB transmission rate among homeless populations, in congregate settings such as shelters, prisons and jails, high immigration rates of TB-endemic countries to San Mateo communities, and a general reduction in the national public health infrastructure supporting TB control activities continue to be a challenge for San Mateo County and throughout the state of California.

 

DISCUSSION:

TB cases and their contacts are typically followed for six to twelve months and sometimes longer. Funding from this award will be used to offset the cost of 1 FTE Communicable Disease Investigator, 2.25 FTE Community Workers, and .33 FTE Social Work Supervisor. The TB staff will provide prevention and control measures including case management, contact investigation, and directly-observed therapy.

 

This agreement is coming to the Board late due to delays in federal funding authorization, which prevented CDPH from sending the agreement until late July 2020.

 

The resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).

 

The agreement and resolution have been reviewed and approved by County Counsel as to form.

 

It is anticipated that approximately 54 active TB cases will be case managed by County Health’s TB Control program to ensure that those patients are no longer infectious, with exposed contacts investigated and managed accordingly.

FISCAL IMPACT:

The term of the agreement is July 1, 2020 through June 30, 2021. The amount of the agreement is not to exceed $347,499 for the one-year term. These funds are included in the Public Health, Policy, and Planning FY 2020-21 Recommended Budget. Since the County is receiving these funds and no matching funds are required, there is no Net County Cost associated with this agreement.
