BACKGROUND:
On November 13, 2018, the Board approved an agreement with Voices of Recovery San Mateo County (VORSMC) for consumer-operated self-help and peer-support services and wellness and recovery services (WRAP) for the term July 1, 2018 through June 30, 2020, in an amount not to exceed $597,787.
On September 24, 2019, the Board approved the FY 2019-20 Adopted Budget. Previously, on June 25, 2019 the Board received the FY 2020-21 Preliminary Recommended Budget. These budgets included a 4% Cost-of-Living-Adjustment for Behavioral Health and Recovery Services Community-Based Organizations that provide mental health services and substance use disorder services.
On September 19, 2019, the Chief of San Mateo County Health approved an amendment to the agreement for peer-recovery supports and community integration, increasing the amount by $25,000 to $622,787, with no change to the agreement term. Previously, these services were in the HealthRIGHT 360 agreement serving as the fiscal agent and VORSMC providing the services.
DISCUSSION:
VORSMC provides consumer-operated self-help and peer-support services for individuals with co-occurring substance use and mental health disorders, and support through a total wellness program. Services include WRAP groups to formerly incarcerated adults through the Criminal Justice Realignment Act, Drug Court clients, Whole Person Care (WPC) clients and other community members as they transition to the aftercare phase of treatment. WRAP is a self-designed plan for staying well, increasing personal responsibility, and improving the quality of daily life.
Through this amendment, the following changes will occur: 1) removal of the Parent Project curriculum in FY 2019-20; 2) add Health Ambassador WRAP Groups; 3) add the remainder of funding to continue providing peer recovery supports and community integration for the prevention of relapse previously included in the HealthRIGHT 360 agreement; and 4) add a 4% cost-of-living adjustment in FY 2019-20.
The resolution contains the County’s standard provision allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).
The amendment and resolution have been reviewed and approved by County Counsel as to form. This amendment is late due to prolonged contract negotiations with VORSMC.
Participation in peer support activities is an indicator of recovery improvement and clients’ commitment to their own wellness and recovery. An increase in attendance indicates that the contractor is serving a larger audience and successful in reaching an underserved population. In FY 2018-19, 44 WPC clients attended peer support/recovery activities and in FY 2019-20 an additional 22% or 10 new WPC clients attended these activities. WPC clients are specifically identified and measured separately from other VORSMC clients.

FISCAL IMPACT:
The term of the agreement is July 1, 2018 through June 30, 2020. The amount of the amended agreement is not to exceed $798,741. The amendment increases the contract maximum by $175,955 and is included in the BHRS FY 2019-20 Adopted Budget. The total amount in BHRS FY 2019-20 Adopted Budget is $490,848. Of this amount, $185,119 will be funded by the State Department of Health Care Services through the Whole Person Care grant, $144,800 is funded by Mental Health Services Act, $40,170 will be funded by AB109, and $87,429 will be funded by 2011 Realignment. The remaining $33,330 will be funded by Net County Cost.
