BACKGROUND:
Measure K is the half-cent general sales tax initially approved by San Mateo County voters in November 2012 and extended in November 2016 for a total of thirty years.
On February 12, 2019, the Board of Supervisors (Board) held a study session on Measure K expenditures and anticipated revenue for fiscal years (FY) 2019-21. The Board-appointed Measure K sub-committee, consisting of Supervisor Pine and Supervisor Canepa, convened the study session to, among other things, work with staff to facilitate the Board’s development of Measure K priorities and continuing initiatives for FY 2019-21. The Board approved $3.75 million in one-time loans for grants each fiscal year for FY 2019-21, divided equally among the five supervisorial districts, for district-discretionary needs and projects. District 5 has submitted a request to use its district-discretionary Measure K funds as shown below and described in the Project Summary section of this memorandum.

PROJECT SUMMARY:
This is a request to authorize a grant to Holy Child and St. Martin Episcopal Church and resulting grant agreement in an amount not to exceed $20,000 to expand their wellness program and purchase additional medical equipment. The County Manager’s Office will administer and manage the proposed agreement.
The Health and Wellness Center at Holy Child and St. Martin Episcopal Church in Daly City was born in 2010 out of the need to serve the whole community by nurturing physical, emotional, and spiritual health, to empower those on the margins, to advocate for health care services, to provide health education, and to partner with other community organizations to create similar programs. In subsequent years, the program served to screen for high blood pressure, to provide information about nutrition and healthy weight, and to sponsor an arthritis exercise program. Each year, a Health and Wellness Fair was presented to welcome the community to the program, educate the community about available services, and to provide free influenza vaccines. Holy Child and St. Martin Episcopal Church previously received a grant from Seton Medical Center, but due to the sale of Seton, community grants have been stalled and that source of funding is not available at this time. The Wellness Center is open to anyone who wants to access services and is not restricted based on any type of religious affiliation. Measure K funds would be solely used toward the physical health and wellness program and would not be used for religious activities.
The Health and Wellness Center at Holy Child and St. Martin Episcopal Church would like to expand their work. In addition to screening for high blood pressure, checking weight, and participating in exercise programs, the Health and Wellness Center wants to provide education regarding the environmental and behavioral factors that play a part in the development of medical conditions, particularly diabetes, and the behaviors and lifestyle changes that will enhance health and wellness.
Program goals include:
• Providing education to clients to become more knowledgeable about behaviors that can lower blood pressure
• Providing assistance to clients in accessing their prescribed medications
• Assisting clients in the proper use of home medical devices, such as automatic blood pressure cuffs and glucometers
• Providing educational information to clients about their diseases and diagnoses, procedures, and medications
• Offering all services free of charge to the community
• Providing a place where the most vulnerable, underserved people of all ages- the homeless, seniors, and families without insurance- can receive free screenings and educational information to assist in maintaining and enhancing health
• Providing group exercise and skeletal improvement programs
• Providing a yearly health fair
• Providing a monthly forum with health educators/doctors that is open to the community

County Counsel has reviewed and approved the resolution as to form.
Funding for the Holy Child and St. Martin Episcopal Church Health and Wellness Center contribute to the Shared Vision 2025 outcome of a healthy community by further educating the community about the importance of a healthy lifestyle, including physical activity, blood pressure checks, and nutrition and exercise programs in order to reduce the risk of chronic diseases and promote overall health.
FISCAL IMPACT:
There are sufficient Measure K funds for this specific FY 2019-2021 Measure K request. These funds are budgeted in the Non-Departmental Services FY 2019-21 Adopted Budget.
