BACKGROUND:

San Mateo County is responsible for providing necessary medical care to its medically indigent population. This care includes urology related services, such as treating disorders of the kidney, urethral tract, and bladder. Dr. David Martin has served as San Mateo Medical Center’s (SMMC) Urology Medical Director since October 2012.

 

DISCUSSION:

Dr. Martin has agreed to continue as Urology Medical Director, in addition to providing four half-day urology clinic blocks per week and two half-day surgery blocks per week.

 

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

 

The agreement and resolution have been reviewed and approved by County Counsel as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

The agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing County residents access to urology services. It is anticipated that 100 percent of all patients diagnosed with kidney stones will receive lithotripsy treatment within 24 hours of the diagnosis.

FISCAL IMPACT:

The term of the agreement is October 1, 2019 through September 30, 2021. The amount of the agreement is not to exceed $1,250,000 for the two-year term. Funds in the amount of $468,750 are included in the SMMC FY 2019-20 Recommended Budget. Similar arrangements will be made for future years.

 

This contract’s payment provisions differ from that of the previous contract. Specifically, there is 18% increase over two years, due to increased urology rates published by the Medical Group Management Association, whose rates SMMC uses to benchmark physician contract compensation. Additionally, call coverage rates have also been increased to market level.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC, and is within the existing annual appropriation.
