BACKGROUND:

San Mateo County is responsible for providing necessary medical care to its medically indigent population. This includes services for cardiology disorders and the management and supervision of those services.

 

DISCUSSION:

Dr. Aparna Bhagavat has served as a non-invasive cardiologist at San Mateo Medical Center (SMMC) since September 2011 through the San Mateo Multispecialty Physician Group. She has agreed to continue in that capacity as an independent provider. She will provide (i) a minimum of five half-day clinics per week, with each such clinic being four hours in length, for 48 weeks per year; (ii) inpatient cardiology consults; (iii) 100% of the Transesophageal Echocardiogram interpretations of SMMC, as well as at least 50% of the readings for Electrocardiograms, Stress Echocardiograms, Stress Tests, and Holter Monitors; and (iv) call coverage for 26 weeks per year.

 

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

 

The agreement and resolution have been reviewed and approved by County Counsel as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).


The agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing basic neurology services to SMMC patients. It is anticipated that wait times for all cardiac studies will decrease by 50 percent.

FISCAL IMPACT:

The term of the agreement is July 1, 2019 through June 30, 2020. The amount of the agreement is not to exceed $570,000 for the one-year term. Funds in the amount of $570,000 are included in the SMMC FY 2019-20 Recommended Budget.

 

The payment provisions have decreased 10% from the previous agreement due to lower 2018 rates as published by the 2018 Medical Group Management Association, which SMMC uses to benchmark its physician contract compensation.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care is covered by the County’s General Fund contribution to SMMC, and is within the existing annual appropriation.
