BACKGROUND:
For over 20 years, Behavioral Health and Recovery Services (BHRS) has contracted with The Latino Commission (TLC) for outpatient and residential substance use disorder treatment and recovery services, as well as transitional housing services. Examples of these services include: assessment, recovery or treatment planning, individual and group counseling, and case management.
In April 2016, the California Department of Health Care Services (DHCS) and the Federal Government approved the Drug Medi-Cal Organized Delivery System (DMC-ODS) waiver for substance use disorder treatment services. In March 2017, TLC began providing services under the DMC-ODS waiver, which allowed TLC and other providers to bill for many more services than they were previously able.
DISCUSSION:
Through the DMC-ODS waiver, TLC continues to provide expanded outpatient and residential substance use disorder treatment services for men, women, and young mothers with children. Services include: assessment, recovery or treatment planning, psycho-education, process and support groups, individual counseling, case management, and financial coaching.
DHCS continues to require changes in service delivery and reporting requirements of substance use disorder treatment services through the DMC-ODS waiver. Due to these changes, BHRS is requesting that your Board waive the Request for Proposals (RFP) process for substance use disorder treatment services for two years, allowing BHRS time to finalize the implementation of DMC-ODS waiver-related changes to this and all other BHRS substance use related contracts. In early 2020, BHRS will release a RFP for substance use disorder treatment services to start on July 1, 2021.
The resolution contains the County’s standard provision allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate).
The agreement and resolution have been reviewed and approved by County Counsel as to form.
Approval of this agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing individuals and families with prevention and treatment services to support recovery, which in turn contributes to the health and safety of communities throughout the County. Successful treatment discharge occurs when a program participant completes their treatment/recovery plan or when a participant is transferred to the next appropriate level of care. It is anticipated that 60 percent of clients will achieve a successful treatment discharge.

FISCAL IMPACT:
The term of the agreement is July 1, 2019 through June 30, 2021. The amount of the agreement is not to exceed $2,077,450 for the two-year term. Of this amount, $1,038,725 will be included in the BHRS FY 2019-20 Recommended Budget. Federal Financial Participation for Drug Med-Cal will fund $309,849, Substance Abuse and Prevention Treatment Block Grant will fund $286,310, 2011 Realignment will fund $108,909, and AB 109 will fund $7,466. Net County Cost will fund $326,191. The payment provisions and levels of service remain essentially the same as those of the prior agreement.
