Adopt a resolution authorizing an agreement with Sadhanandham Tivakaran, MD to provide gastroenterology services for the term of April 1, 2019 through March 31, 2020, for an amount not to exceed $210,000.

BACKGROUND:
Dr. Tivakaran has been providing gastroenterology (GI) procedure services, primary endoscopy, for San Mateo Medical Center (SMMC) since October 2018. With the aging population of SMMC’s patients, there has been a dramatic increase in the medical need for endoscopy services.
DISCUSSION:
Under the terms of the agreement, Dr. Tivakaran will provide two days per week of GI procedures to meet the current demand and address the back log of patients requiring this service.
County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.
The agreement and resolution have been reviewed and approved by County Counsel as to form. The contract is late due to prolonged negotiations with Dr. Tivakaran.
The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).
The agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing San Mateo County residents with access to basic GI services. It is anticipated that the wait list for GI procedures will be reduced by 30%.

FISCAL IMPACT:
The term of the agreement is April 1, 2019 through March 31, 2020. The amount of the agreement is not to exceed $210,000 for the one-year term. Funds in the amount of $52,500 are included in the SMMC FY 2018-19 Recommended Budget. Funds in the amount of $157,500 will be included in the SMMC FY 2019-20 Recommended Budget.
The payment provisions and levels of service in this contract are essentially the same as the prior contract.
Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care are covered by the County’s General Fund contribution to SMMC, and are within the existing annual appropriation.
