BACKGROUND:
The San Mateo County Health System has been awarded the Health Care for the Homeless/ Farmworker Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services. The grant funds San Mateo Medical Center's (SMMC) ongoing HCH/FH program, which is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC's continuum of care and other medical, health and support services. HCH/FH has contracted with Samaritan House since 2014, to provide enabling services for the homeless.

DISCUSSION:
In September 2017, HCH/FH issued a Request for Proposals to providers to deliver health and health related services to the homeless and farmworker populations of San Mateo County. Samaritan House was chosen by the HCH/FH Program Co-Applicant Board for its innovative approach to providing care coordination/enabling services for the homeless population in the San Mateo County. These services include outreach, patient and community education, transportation, follow-up, translation services, referral services and ongoing support to improve client access to San Mateo County Health System's primary medical services and HCH/FH Program contractors.
