B)                     The Chief of San Mateo County Health or her designee to execute agreements with Alzheimer’s Disease and Other Related Diseases, Inc., Daly City Peninsula Partnership Collaborative, Edgewood Center for Children and Families, Institute on Aging, and Ombudsman Services of San Mateo County, Inc., for the term of July 1, 2019, through June 30, 2020, in an amount not to exceed $1,008,635.

BACKGROUND:

On April 28, 2015, the Board of Supervisors, as recommended by the Measure K Subcommittee, approved continuing and/or expanding initiatives that were part of the FY 2013-15 funding cycle, as well as new initiatives and projects for FY 2015-17. Alzheimer’s Disease and Other Related Diseases, Inc. (ADRD), Daly City Peninsula Partnership Collaborative (DCPPC), Edgewood Center for Children and Families (ECCF), Institute on Aging (IOA), and Ombudsman Services of San Mateo County, Inc (OSSMC) were selected as new initiatives and projects under the Older Adults and Veteran Services priorities.

 

On December 13, 2016, the Board of Supervisors, as recommended by staff, approved these recommended Measure K allocations for FY 2017-19. On June 6, 2017, and June 5, 2018, your Board approved contracts for ADRD, DCPPC, ECCF, IOA, and OSSMC for the term July 1, 2017, through June 30, 2018, under Resolution #075222 and for the term July 1, 2018, through June 30, 2019, under Resolution #075920.

 

On February 12, 2019, the Board of Supervisors, as recommended by staff, approved continuing funding for these ongoing Measure K initiatives for FY 2019-21.

 

DISCUSSION:

San Mateo County Health requests that the Board adopt a resolution authorizing the Chief of San Mateo County Health or her designee to execute agreements to continue the following Measure K funded services through the providers listed below. The term for the new agreements with these providers will be July 1, 2019, through June 30, 2020.

 

Daly City Peninsula Partnership Collaborative - Friendly Visiting and Meals Express program ($146,904)

The Daly City Peninsula Partnership Collaborative’s (DCPPC) Friendly Visiting and Meals Express program provides services to alleviate the isolation felt by older adults and persons with disabilities, while maximizing their ability to age in place. DCPPC will serve emergency meals, grocery or “brown bag” delivery assistance to homebound seniors or disabled adults who have recently been discharged from the hospital and do not immediately qualify for home-delivered meals, or have been placed on a waiting list for home-delivered meals service.

 

Edgewood Center for Children and Families - Kinship Caregiver Mental Health Counseling ($77,250)

Edgewood Center for Children and Families will expand its Kinship Caregiver Counseling Program by offering additional mental health services for senior kinship caregivers and implement an outcome tracking system utilizing the Adult Needs and Strengths Assessment and CSQ-15 Satisfaction Survey to measure the effectiveness of those services.

 

Institute on Aging - Friendship Line ($206,000)

The Institute on Aging’s Friendship Line (Friendship Line) provides services to older adults and adults with disabilities in San Mateo County. The Friendship Line is the only 24-hour hotline in the U.S. offering suicide intervention and prevention for seniors and disabled adults, and provides crisis support services that include active suicide intervention and a unique call-out service.

 

Alzheimer’s Disease and Other Related Diseases, Inc. - Dementia Capable Services and Supports ($463,500)

The Dementia Capable Services and Supports (DCSS) initiative will focus on the needs of persons with Alzheimer's disease and related dementias (ADRD) living alone in San Mateo County. The goals of the DCSS initiative include: (1) building a coalition of dementia-capable organizations and County agencies to affect systems change; (2) training and educating providers (medical, social, and volunteer) about ADRD and its impact on the individual living alone; and (3) developing, delivering, and evaluating multidisciplinary services to meet the unique needs of persons with ADRD living alone.

 

Ombudsman Services of San Mateo County, Inc. ($114,981)

The Ombudsman Services of San Mateo County, Inc. (OSSMC) is a critical component of the County’s effort to support those who are most vulnerable and at-risk. OSSMC will provide onsite monitoring and complaint investigation and resolution services to individuals residing in San Mateo County long-term care facilities.

 

A waiver for the RFP process is requested for these agreements because staff have determined that the selected provider is the sole source for the highly specialized, unique services provided pursuant to each agreement.

 

The agreements and resolution have been reviewed and approved by County Counsel as to form. The resolution contains the County’s standard provisions allowing amendment of the County’s fiscal obligations by a maximum of $25,000 (in aggregate per agreement).

 

The resolution also contains a provision to allow the Chief of San Mateo County Health or her designee to modify the performance measures to implement changes in the reporting requirements, as may be appropriate to help the County more effectively communicate information about Measure K programs.

 

These agreements contribute to the Shared Vision 2025 outcome of a Healthy Community by providing specialized community-based services to allow older adults and persons with disabilities to continue to live independently in the community. It is anticipated that 41,000 individuals (aged 60 and above) will receive home and community-based services through Aging and Adult Services (AAS) contracted community-based providers.

FISCAL IMPACT:

The term of the agreements is July 1, 2019, through June 30, 2020. The total amount of Measure K funds included in the above contracts for FY 2019-20 is $1,008,635. Funding for these agreements will be included in the AAS FY 2019-20 Recommended Budget.
