BACKGROUND:

San Mateo County is responsible for providing necessary medical and dental care to its medically indigent population and access to services to its general population. Dr. Oka has been providing oral and maxillofacial surgery services at San Mateo Medical Center (SMMC) since May 2008 and will continue to provide, at a minimum, the same level of service as he has under previous contracts with the County.

 

DISCUSSION:

SMMC has a continuous need for oral and maxillofacial services due to the County’s aging population. SMMC will continue to meet this need through contracts with Dr. Oka and other dental surgeons. Dr. Oka has agreed to provide necessary surgery, clinic, and call coverage for SMMC patients.

 

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

 

The agreement and resolution have been reviewed and approved by County Counsel as to form.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

The agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing access to oral and maxillofacial surgery services to County residents. It is anticipated that 100% of all oral and maxillofacial surgery services provided will adhere to the infection control standards as defined by the Joint Commission and Title XXII of the State of California, Code of Regulations, which will result in reduced risk of healthcare-associated infections.

FISCAL IMPACT:

The term of the agreement is May 1, 2019 through April 30, 2021. The amount of the agreement is not to exceed $150,000 for the two-year term. Funds in the amount of $12,500 are included in the SMMC FY 2018-19 Adopted Budget. Funds in the amount of $75,000 will be included in the SMMC FY 2019-20 Recommended Budget.

 

The payment provisions have decreased 33% from the previous agreement due to an adjustment in the not-to-exceed amount of the contract to better align with the actual amount of services billed in previous fiscal years by Dr. Oka.

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care are covered by the County’s General Fund contribution to SMMC, and are within the existing annual appropriation.
