BACKGROUND:

San Mateo County is responsible for providing necessary medical care to its medically indigent population. This includes professional specialty orthopedic services and the management and supervision of those services. Duc Marcel Nguyen, MD has provided professional specialty orthopedic services at San Mateo Medical Center (SMMC) for the past seventeen years.

 

DISCUSSION:

The County’s need for orthopedic services continues to increase. SMMC will be able to meet this need through contracts with Dr. Nguyen and other orthopedists. Dr. Nguyen will continue to provide, at a minimum, the same level of services as under the previous agreement.

 

County Administrative Memorandum B-1 provides that contracts for physicians are exempt from the Request for Proposals process.

 

The agreement and Resolution have been reviewed and approved by County Counsel as to form.

 

The Resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

The agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by providing access to specialty orthopedic services to residents of San Mateo County. SMMC is acutely aware of the importance of controlling hospital borne infections. Failure to control infection results in higher costs to the County for having to take additional care of those patients who become victims of a preventable infection. It is anticipated that 100% of all orthopedic surgery services provided at SMMC will adhere to the infection control standards as defined by the Joint Commission and Title XXII of the State of California Code of Regulations, which will result in reduced risk of healthcare-associated infections.

FISCAL IMPACT:

The term of the agreement is April 1, 2019 through March 31, 2021. The amount of the agreement is not to exceed $600,000 for the two-years term. Funds in the amount of $75,000 are included in the SMMC FY 2018-19 Adopted Budget. Funds in the amount of $300,000 will be included in the SMMC FY 2019-20 Recommended Budget.

 

The payment provisions and level of services in this contract are essentially the same as those of the previous contract.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care are covered by the County’s General Fund contribution to SMMC, and are within the existing annual appropriation.
