BACKGROUND:

The Health System was awarded a HealthCare for the Homeless /Farmworkers Health (HCH/FH) grant from the Bureau of Primary Health Care, Health Services and Resources Administration of the US Department of Health and Human Services. The HCH/FH grant funds San Mateo Medical Center (SMMC)’s ongoing HCH/FH Program, which provides primary medical, dental, and behavioral health care to San Mateo County’s homeless population. SMMC’s HCH/FH Program is designed to assist homeless and farmworker adults, families, and youth with access to and utilization of SMMC’s continuum of care and other medical, health and support services. The HCH/FH Program contracts with both County departments and community-based organizations to provide this care. The HCH/FH Program has contracted with LifeMoves since 2008 to provide care coordination/ support services for the homeless.

 

DISCUSSION:

In September 2017, HCH/FH issued a Request for Proposals to providers to deliver health and health related services to the homeless and farmworker populations of San Mateo County. LifeMoves was chosen by the HCH/FH Program Co-Applicant Board for its innovative approach to providing care coordination/enabling services for the homeless population in the San Mateo County. These services include outreach, patient and community education, transportation, follow-up, translation services, referral services and ongoing support to improve client access to San Mateo County Health System’s primary medical services and HCH/FH Program contractors. In addition, LifeMoves will provide care and transportation coordination with the San Mateo County Public Health Street Medicine team.

 

The agreement and resolution have been reviewed and approved by County Counsel as to form. The agreement is coming to your Board late due protracted negotiations.

 

The resolution contains the County’s standard provisions allowing amendment of the County fiscal obligations by a maximum of $25,000 (in aggregate).

The agreement contributes to the Shared Vision 2025 outcome of a Healthy Community by
providing health care services to homeless individuals residing at shelters, to unsheltered homeless individuals residing on the streets, and farmworker individuals of San Mateo County. It is anticipated that a minimum of 690 unduplicated homeless individuals will receive care coordination and other enabling services.

FISCAL IMPACT:

The term of the agreement is January 1, 2018 through December 31, 2018. The amount of the agreement is not to exceed $298,030 for its one-year term. Funds in the amount of $149,015 are included in the SMMC FY 2017-18 Adopted Budget. Funds in the amount of $149,015 will be included in the SMMC FY 2018-19 Recommended Budget.

 

Previously, SMMC contracted for care coordination and enabling services separately. This agreement combines these services into one agreement. Compared to the previous agreements, this agreement represents a 16% increase due to the addition of increased transportation coordination for homeless individuals to travel to/from their appointments.

 

Expenses at SMMC are covered by fees for services or third-party payors whenever possible. The portion of expenses for services provided to the medically indigent or to those covered by programs that do not meet the full costs of care are covered by the County’s General Fund contribution to SMMC, and are within the existing annual appropriation.
